- GREATER LOS ANGELES AREA - HOME HEALTH AGENCY INCLUDES AREAS SUCH
AS HAWTHORNE, COMPTON, SANTA FE SPRINGS, BELLFLOWER, DOWNTOWN LOS
ANGELES, AND THE GREATER LA AREA Spanish speaking preferred, but
not required THIS POSITION REQUIRES TRAVEL TO PATIENT HOMES. PLEASE
BE AWARE THAT THIS IS NOT AN 8- OR 12-HOUR POSITION IN A FACILITY
OR NURSING HOME THIS POSITION IS PAID PER VISIT BASED UPON THE
NUMBER OF PATIENTS ACCEPTED Mileage compensation: to be discussed
Type of employment: PER DIEM Compensation: $100 for evaluation,
re-evaluation, and discharge visits. $65 for follow up visits. We
are currently seeking a Licensed Physical Therapist (PT) to travel
and visit patients in the Los Angeles and surrounding areas. We
have a high number of patients all throughout Southern California
and have plenty of work available. This position is perfect for
therapists who are seeking a flexible role that works on their own
schedule. PT will be primarily responsible for evaluations,
re-evaluations and discharges, but are more than welcome to conduct
follow up visits as well. QUALIFICATIONS: Is a graduate from a
physical therapy curriculum approved by the American Physical
Therapy Association or the Committee on Allied Health Education and
Accreditation on the American Medical Association or the Council on
Medical Education of the American Medical Association and the
American Physical Therapy Association. Current license from the
Physical Therapy examining Committee of the California Board of
Medical Quality Assurance. Possesses at least one (1) year of PT
experience in a clinical setting; home health or geriatrics
preferred but not required specifically. Current CPR certitifcation
Is able to walk, bend, stoop, and lift objects up to 50 lbs Current
and satisfactory report on pre-employment physical examination
including Mantoux TB test as required by Agency and State policies
and procedures. Is fluent in English - additional language fluency
is preferred but not recommended. Has excellent judgment and
knowledge of current therapy practices, with demonstrated exemplary
clinical, analytical and documentation skills. Is self-directed and
able to work with little supervision and has good organizational
skills. Must be a licensed driver with an automobile that is
insured and in accordance with state and/or organizational
requirements and is in good working order. POSITION SUMMARY The
Physical Therapist organizes and conducts medically prescribed
physical therapy programs for patients to restore function and
prevent disability following disease, injury or loss of a body
part; helps the patient reach maximum performance within the limits
of his/her capabilities. ESSENTIAL DUTIES AND RESPONSIBILITIES The
following is a representation of the major duties and
responsibilities of this position. The Agency will make reasonable

disabilities to perform essential functions. 1. Will provide
physical therapy services in accordance with a plan oftreatment and
plan of care. 2. Provides treatment as ordered by the attending
physician, dentist, or podiatrist. 3. Assesses the therapeutic and
rehabilitative status of the patient and develops appropriate
treatment plan for each patient; updates and modifies treatment
plan on an ongoing basis. 4. Evaluates the home environment and
makes appropriate recommendations on safety and the use of medical
devices. 5. Assists the physician in evaluation level of function.
6. Plans, goals and care are defined that are reasonable, necessary
and specific for the skills ofthe therapist. 7. Performs required
treatments and adapts conventional treatment techniques to meet the
needs of the patient unable to comprehend verbal commands. 8.
Observes and evaluates treatment effect and patient's overt
physical/emotional response to treatment goals. Updates treatment
plan based on evaluation and as new orders are received. 9.
Coordinates care with the attending physician, Case manager and
other disciplinary team members, and the patient/caregiver. 10.
Contacts physician for clarification or elaboration of treatment
orders, or to alert to any significant changes, and to discuss
condition and treatment course of patient making recommendations
for changes as needed. 11. Advises, consults and may instruct
family and Agency personnel concerning patient's therapy program.
12. Teaches and supervises other health personnel when appropriate.
13. Observes, records and reports information on the patient's
condition to the attending physician and in the patient's medical
record. 14. Advises, consults and when appropriate, instruct family
and other agency personnel, in patient's therapy program. 15.
Evaluates the home environment and makes appropriate
recommendations. 16. Instructs the patient and/or caregiver in
proper exercise techniques and in the use of all applicable
devices. 17. Documents patient treatment and response to treatment
accurately, using established guidelines. Prepares
clinical/progress notes. 18. Submits all paperwork including but
not limited to: assessment, care plan, change of orders, visit
notes, medication records, progress summaries, CHHA supervision,
and transfer/discharge summary within time guidelines according to
Agency policy. 19. Collects data for the Outcomes Assessment
Information Set developed and required by HCF A. Submits data sets
within the guidelines established by HCF A. 20. Regularly
participates in interdisciplinary planning, case conferences and
staff meetings as required. 21. Participates in Performance
Improvement activities including but not limited to clinical record
review, quality indicator monitoring, and quality management
committee. 22. Actively participates in Agency in-service education
programs and/or provides in service related to PT services as
requested. 23. Maintains confidentiality of patient information at
all times. 24. Must participate in professional continuing
education and maintain active license in the State of California.
25. Conforms to all Agency policies and procedures. Company
