Looking For The Patient Services Coordinator, Spiritual Care
manages Spiritual Care patient check-in and check-out duties, greet
patients, answer phones, verify patient demographic information,
helps patients with appointment scheduling and performs assigned
clerical responsibilities. Must manage and work collaboratively
with staff and volunteer chaplains. Provides age and culturally
appropriate spiritual resources to patients, families and staff to
assist chaplains. Supports patients and their families with
educational resources about their spiritual health. Identifies
appropriate services for patients and families and coordinates
scheduling. Removes obstacles to spiritual care treatment by
scheduling appointments, coordinating referrals, arranging
transportation, childcare, translation, and other needed services.
Tracks and documents metrics and outcomes. Manages internal
referrals promptly and efficiently from internal SACHS departments,
ensuring seamless coordination. Schedule : 4days per week, 10 hours
per day Mon - Thur, 7:00 am - 5:30 pm | Location : Brier Clinic,
San Bernardino, CA ESSENTIAL FUNCTIONS AND DELIVERABLES Responsible
for patient check in/out, schedule appointment requests, reschedule
cancellations for assigned Spiritual Care department. Handles
sensitive or confidential information with discretion and sound
judgment, knowing when to make decisions independently and when to
seek input from others. Understand HIPAA expectations, computer
privacy and personal health information documents and follow SACH
policies. Demonstrates the ability to identify the patients account
via date of birth or name search; creates accounts for new patient
appointments; and verifies and updates demographic information.
Knows and follows eligibility requirements and verification
processes for coverage programs. Enters confidential personal
health information into EMR accurately. Complies with federal and
local laws in ensuring patient privacy. Assist in promoting
spiritual care services by engaging with patients and staff across
all locations. Prepare, proofread, and format documents, reports,
and presentations. Work on quarterly newsletters, presentations for
church visits, and conference seminars. Organize conferences and
meetings including arrangements for equipment, logistics, and
catering. Schedule and organize events such as mission trips,
spiritual emphasis week, spiritual renewal week, holiday services,
and special support groups. Order and manage office supplies and
inventory such as bibles, spiritual care books, and religious
resources. Required to visit various SAC Health locations as
needed; must have a reliable vehicle, valid driver's license, and
auto insurance. Other duties as outlined in the official job
description. QUALIFICATIONS: Education: High school diploma or
equivalent required. Bachelor's degree in Communications, or a
related field preferred. Licensure/Certification: Valid CA Driver's
License and Auto Insurance Required. As a requirement of this
position, you must receive EPIC certification for the module you
have been hired into. Experience: One (1) year of customer service,
general office, or healthcare related experience required. Two (2)
years of ministry volunteer work preferred. Essential
Technical/Motor Skills: The ability to use a computer on a routine
basis and complete required documentation. Use of word processor,
spread sheets and internet. Interpersonal Skills: An empathy for
the problems experienced by patients and families; commitment to
help the distressed. The communication skills necessary to counsel
patients, families, and employees. The analytical ability to assess
patients' spiritual needs and sense of well-being and find
appropriate solutions to patient problems. Work Eligibility: Must
be legally authorized to work in the United States on a full-time
basis. Must not now or in the future require sponsorship for
